  • Complete the online nomination form. Provide contact details of the candidate nominee, nominator and contact details of up to three references (sponsors).
  • Contact details of up to three references (also required for self-nominators).
  • A nomination statement describing how the nominee has contributed to one or more of the areas specified in the eligibility criteria below.

 

Required documents

  • Nominations must be accompanied by the following supporting documentation, all documents are required in English:
  • Recent CV of nominee
  • Detailed description of noteworthy contributions, achievements and/or service.
  • Up to three letters of support from sponsors
